help my LPS!!

Have a 10 gallon nano reef. Here's a brief overview:
Lighting: 4100 orbit marine pro 24in (72 3w LED's)
filter: tetra 10 whisper (got it to run carbon) filters are changed bi-weekly
Inhabitants:
-2in scooter blenny
-xenia frag
-brain coral frag
-misc. muchrooms
-GBTA
- small frogspawn frag
this tank has been running for approximately 6 months.
I do 30-40 percent water changes weekly depending on parameters
parameters:
PH: 8-8.2
alkalinity: 8dkh
ammonia: 0
nitrite: 0
Nitrate: .025
phosphates: 0
magnesium: 1125
calcium: 400
salinity: 34

Guys i need help!! I have lost a torch with branching heads and believe i'm about to lose my frogspawn as well. Everything else in my tank is fine! my anemone seems to be a little irritated as he is a tad smaller than normal but is inflated and has a small mouth and no discoloration and everything else looks great. Over the past week i noticed my torch getting darker in appearance and retracting. at first I just left him be as it is normal for him to retract from time to time. after the second day i became very worried and checked my parameters. My salinity had dropped to about 26-27 and figured this was the issue and began to raise it back up over the course of this week. This did not change a thing. after coming home today i noticed he had a white slime mucus around the rim and decided to remove him. when i removed him I noticed two of what i believed to be bristle worms on his skeleton. Now my frogspawn (2.5 weeks old) has shriveled up and the mouth has been open for two days or so and i am very worried im going to lose him aswell :( I thought removing everything including my rock and substrate and doing a deep clean aswell as dipping all of the corals. what are some of yall's thoughts? I am still fairly new to reefing so i'm having a hard time troubleshooting my issue. some sort of lps disease? parasites? help!!!!!
 
Do you have any pictures of the corals in question? Could be a variety of different things.

Euphyllia species (Torches, frogspawn) prefer magnesium at around 1200 - 1350. Yours seems a bit low.
How many mushrooms and how big is the xenia? They might be using it all up.

What salt mix are you using for your water changes?


I thought removing everything including my rock and substrate and doing a deep clean aswell as dipping all of the corals.

Rocks don't need to be taken out and cleaned. The are filled with beneficial bacteria that help with the natural biological aquarium filtration that are responsible for breaking down organic waste within the fish tank. They also serve as homes for various small species of pods that also help with the process.

Cleaning coral is acceptable. Many frags like to grow algae and going in there to remove it with a very gentle brush helps with the overall health of the coral.
 
Seems to me that you are changing way too much water. 10% weekly for the first year is a pretty good change schedule. Every time you take 30-40% of your water out you are taking alot of the good stuff in your water out and your good bacteria can't establish itself as fast as you are changing water. I know it sounds crazy but you water CAN be TOO clean. You may be robbing them of their food source. Also keep your lighting in check. My lps get pretty mad if I go above regular setting which is 45% blues 20% whites. The fact that you have a nano leads me to ask do you have anything else within 3 inches or so of your frogspawn?
 
Your Nitrate and phosphate are both so low, LPS does not seem to be happy with that clean water. I used to keep some LPS in that water parameter, I lost most of them, I raise everything up a bit. The rest are happier now. 0.25 nitrate and 0 phosphate can starve your LPS
 
I agree with danh. You have very low nutrients. Reducing your water change schedual will help with this. Make sure your using a high quality salt in order to maintain proper water chemistry. Your salinity also seems a bit low. I try to keep mine around 35ppt
