It looks like something burned it. Maybe a nematocyst strand from another coral floated over and settled there? Either way, it may very well split bc of the hole. How do the edges inside the hole look (hard to tell in pic)? If the hole is getting bigger but the tissue around the hole looks good I would say it is intent on splitting. As weird as it looks I think it will end up ok.

my Devil's Hand did the same thing after few months of getting it and it recovered fine. Thought it was dying too... like James M said, it may be shedding.

I'd use a turkey baster and gently blow into the hole. That way it'll tell you whether any rotting tissue comes out of it or just detritus. If it's only dirt particles then your toadstool is alright.

Either way I guess I'll just have to wait and see what happens. The tissue around the edge of the hole looks good.

mine had hole just like Loweryphil's image and using a turkey baster some "debris" looking stuff came out and I thought Devil's hand was toast. Was only noticeable at night when it closed. But it healed in few days (maybe a week, dont recall as it was a long time ago) and has been thriving since, I think it was growing. I'd leave it alone and let it heal/do its thing as long as its opening during the day. Or it might be splitting.

I like the turkey baster idea. Looks like something is in there. Isn't there a type of boring crab or such? I would pull it & take a good look into the hole .

Did you discover anything in the Toadstool hole?
Ugh yeah. I blew all sorts out. Stuff that looked like toadstool flesh. It looked like inside the hole was disintegrating, and after blowing it all out, tbe hole went straight though and a rip appeared. Still in my system though I'm hoping it comes back strong

Loads of debris came out! Like actual flesh though.

that's what my Devil's hand did also, even saw a hermit crab eating it so removed the hermit from coral. Mine recovered eventually with hole gone, think it took few days if not a week of being left alone. Is ur Toadstool opening up during day time?

It was before I blew everything out. The little fingers on it come out apart from around the hole, but I think with blowing all that crap out it's sent it into shed mode

as long as it's opening up during daytime it should be ok. I'd leave it alone and keep an eye on it and let it do it's thing.
