Help with disease on Valentini Puffee

Just noticed this yesterday when the puffer didn’t come out to eat. Looks like some sort of fin rot or something. Have had for ~5 months and has shown no signs of anything. He has been very lethargic, just sitting on rocks, which is completely out of the norm. I mean, I was able to catch him with my hand for photos...

Very sad as I absolutely love this fish and his personality.

All water parameters are perfect. Aquarium has been established for over a year, while majority of corals, rock, fish came from my old tank which was established for ~10 years.

I suspect flukes. I would do a FW dip to see if any dislodge.

I see coverage on the fins and I can also see some on the eye.

Ok. I'd strongly consider @HotRocks suggestion to do a freshwater dip to confirm or eliminate flukes.
