I woke up to this

The only thing next to it is my hammer and they have been touching for a long time.
 
It looks to be throwing its mesenterial filaments. Being deflated like that I'd assume warfare from the hammer at this point. Most of the time euphyllia can get along. I'd move it for the time being just to be safe.

That’s weird...they been touching for almost a year now
 
I have bubble tips but there happy and yes a run carbon
 
i moved it away and let’s see if it improves

Most of the times frogspawns and hammers get along but sometimes out of no where, warefare can happen. Even with different types of hammer (ancora, parancora). Try moving it elsewhere away from the hammer with lower flow. Also, have you tested your parameters in case there was a sudden change?
 
The frogspawn is receding a ton of tissue and sees lot of Skelton

Check your parameters incase there has been a change. Sometimes damage from an attack isn't immediate and can take a little time to show it's full extent. Maybe try an iodine dip

Oh no :/ try leaving it in low flow away from other corals. Hopefully boubces back. Sorry to hear. Sucks when this happens for uknown reasons
 
Anything that could have decided to start nipping in the tank? Angel or crab. Or an nem that relocated recently?

no nipping fish and no crabs. My new hasn't moved at all
