Help with my friend's grouper

He has a 7" panther grouper not eating since last 10 days, hes in a 180 with plenty of caves, hermits, hes been there for last two years, shy, using melafix, mostly hides and it lays against the glass

In addition to the background information asked for in the post above, is the grouper breathing fast? If you post a video, I can judge that for you.

Very little info to go on, my best guess would be an internal parasite.

It would be a lot easier to help if you were able to provide additional info - the post in the 1st comment above is a great place to start. There are some experts in this thread already watching, but without additional info it is not possible to give accurate help
 
Sorry, we really need more background information (from the post last night), as well as pictures/video taken under white lights. Even then, it may not be possible to determine what the exact issue is, but without that information, there is no chance for us to give any real advice.

It may well be a chronic issue - Mycobacterium or fatty liver disease. Those are not treatable. More treatable things would be water quality issues (thus needing to know what the water is testing out as) as well as aggression and some parasite issues (but knowing what other fish are in the tank helps diagnose that).

Melafix is rendered useless in a saltwater tank as it is merely teak tree oil and would be regarded as an ineffective tonic. Pics here and even video under white lighting will prove valueable. Knowing the water quality also helpful in particular. . ammonia-ph-nitrate and salinity level as of today.
What foods are being fed and any live feeders ?

That's a good point - if the fish has mostly been fed feeder fish that are high in thiaminase, it could be deficient in thiamin. However, that isn't correctable once a fish has stopped feeding....ugh.
