Help with my Leopard Wrasse?

I just noticed my wrasse looks off.

Fins are torn, and chunks of her scales seem to be missing? its hard to tell, they're white-ish and does not look like fungus or bacteria. A lot of scales, looks to be too much for a fight, towards her tail on her sides.

She's eating, and swimming without issues as you can see in the video.

water params are stable and normal. Temp may be higher than usual since it was very warm today.

Any ideas whats going on?

 
Few more pics. On the other side of her body it looks like her spots are a mess. Is she transitioning?

EDIT: if it makes a difference, she's been hanging close to my flame wrasse when I am not close to the tank. When I get close, she'll swim for attention as usual
