You still need to treat the sailfin despite the other fish being fine. Treating external parasite is much easier in a QT. Many meds’ effectiveness is greatly reduced due to the living bacteria in the DT. I’ve never seen anything this bad, but I would suggest to start with api general care.

Welcome to Reef2Reef, Rez ... even though it's not in the best of circumstances!

I've never encountered what I'm seeing on this tang (he looks healthy and well-fed, in spite of what appear to be parasites of some sort!), so hoping @Humblefish stops by to take a look. Treating this without medicines could be a fairly significant challenge; I'd suggest to keep the water (hyposalinity or otherwise) as clean as possible, and perhaps to supplement your fish's diet with vitamin formulations, fish oil, varieties of algae or seaweed you might have access to - anything that might help boost this fellow's immune system.

~Bruce, hoping you find success!

Agreed. It looks like serious flukes or something alien to this world to me! Wow!

Sorry I am not more help!

Fish lice? Many meds cover this but I’ve never seen one in person.
