Help with Tomini Tang fin issues

I have had this little tomini tang in qt with 2 juvi ocellaris clowns for about 5-6 weeks. The only medication that I have added was prazipro and I finished the second round of that a couple of weeks ago.

Bio media was taken from an established tank, readings are as follows:
Ammonia 0
Nitrite 0
Nitrate 0
Salinity 1.025
Temp 78

A small amount of carbon in run in the tank 24/7

So far everything everything has seemed fine, all fish were eating fine (as of yesterday) and I never saw any aggression between any of the fish.

I woke up today and found him like this.

I made a quick diy acclimation box and got him in there asap.

2-3 days ago I noticed his mouth was black inside, I didn't think much of it of the time but idk if maybe it is related. I also noticed he seemed very lethargic (I literally just picked him up and but him in the box) and his breathing is a little heavy.

It looks more than just nipped fins to me, but I have never had this kind of problem so I could use some help.

Definitely looks like he's getting bullied. I would start running some antibiotic to make sure those edges don't get infected.
 
Unfortunately he was dead by the time I got back from the lfs with meds. I have never seen a fish go from seemingly healthy to dead in such a short time, especially after being in qt for so long. :(

It would be strange if he was getting bullied. This tank is in my office where I work 80% of the time. I have never observed bullying, not even today when he was just sitting at the bottom of the tank.

Couldn't have got caught in a powerhead, could he?

I really don’t think so but I guess anything is possible.
 
Two things in this hobby that can kill a fish incredibly fast: velvet & gram negative bacterial infection. Yours was probably the latter. Sometimes by the time symptoms show, it's already too late to help the fish's immune system. Only way to be sure would to do a scrape and stain test. Sorry for your loss. :(

I haven't had a bacterial infection in any of my fish in years, I didn't realize they could kill so quickly. Is it common for them to be able to show up after weeks of nothing being added to the tank?

Bacterial infections are secondary infections and can be caused by almost any injury, illness, tank conditions, etc. Here's a good read on bacterial infections by our own Humblefish. http://www.reefedition.com/tank-terror-bacterial-infections-reef-tank/
