Ammonia 0ppm
Nitrite 0ppm
Nitrate 0ppm
Phosphate .25ppm
ph 8.2
KH 143.2ppm
Calcium 520
Salinity 1.025
Mg is the only one i cant test

How close are those polyps to the base? As a newbie with SPS I have had STN from the base happen when I had big alk swings and with one acro that was just not getting enough light for a long period. (I think)

Zero nitrates with slightly elevated phosphates may not be a great combination. (It's the only thing that really jumps out at me, but SPS do like to have _some_ nitrate available for snacking.)

Looks like slow tissue necrosis. What temp do you keep your tank at?

Id check your flow around the base of the coral too, maybe its getting blocked by something and causing the coral to STN like that.

look at your tank at night with a red light. Looking for fire worm type thingies.
