Hippo in trouble

I have had this poor little guy for a year, he took a whole month to treat for ick when I first got him. Year later he comes down with some sort of black ick. I moved him to a quarantine and wondering what I should do.

He was in a 40 reef. He was tank mates with a pair of Occelarris and a yellow wrasse
