I agree cheap carbon is one cause of HLLE. This is why I only use BRS Rox 0.8.
 
I'm using Red Sea at the moment to get some meds out of the water. Decent quality?
 
Brs does free shipping. I use the rox .8 carbon. Does a great job.

+1 I haven't used any other carbon in years now. And I'm no BRS fanboy, and I like to save money wherever I can. But this is one thing I don't fool around with; the way I see it, I don't save sick fish and spend months sometimes reconditioning them only to see them literally get cut up by some cheap carbon dust. o_O
 
I've had similar experiences with Rox carbon as well. I suspect it was aggravated by insufficient rinsing of carbon allowing dust to circulate. I was also recommended to lightly 'massage' carbon periodically by the Zeovit regime. I think that contributed to my dust problem. I would also recommend passive use where water flows around versus forced through to minimize dust generation. I also recommend, as does the article linked, a form of post-filtration to capture dust particles. Fwiw, the HLLE reversed completely in a Purple Tang after the rox carbon was removed along with aggressive and varied foods.
