So at first I thought brook, than ich , and now velvet.. I'm but confused.
I will have qt next week hopefully..
Parameters are fine except nitrate that's I decrease
Amo 0
Nitrite 0
Nitrate 50
Salinity- 1.025
He is eating very well. Finds a good spot and rest . He was flickering sometimes grinding on the side on the sand, seems to be less doing it.
I'm feeding with garlic energizer 2-3 times a day I heard it help for their immune to ich / disease .
Help me to find out what is it

That looks like late stage ich to me.
It isn’t something fish can be immune to - it’s an external parasite that feeds off the fish. It doesn’t work the same way as a viral or even bacterial infection would.
Proper treatment is to qt in copper (coppersafe or copper power) and leave the display tank fallow for 60+ days.
Hyposalinity is also an option in a system with no inverts.
 
That fish looks like brook to me. Clowns can fight it off longer then other fish but eventually succumb.
 
I have an skunk shrimp in dp , 2 bristle stars and another clown, that have the symptoms but does not have any of the white spots on him .

These are secondary bacterial lesions associated with brook and may be mixed with ich but start with typical treatment for brook which is a ruby rally pro bath and then treatment in a separate tank
While ruby is reef safe, you will want isolation from any other occupants and leave clown out of display tank for at least 4-6 weeks
No need for garlic 3x a day and disregard nitrites which is freshwater specific
 
Looks like you’ve gotten three different diagnoses here. Seeing a video taken under white light would help rule some of those out.

Any other fish in the tank? How are they doing?

Only another clown and skunk shrimp.
The black clown is fine? I'm not sure, he had some strange behavior to but not as the orange one.
The orange clown starting to look very very white.. pale...
I don't think he will make it thru tonight...
I will have a qt tank just next week and lfs Gere does not have the ruby reef , they will get it more few days.
I'm really.feelimg bad for him I don't have anything to do right now ..
Do a big water change will help after I will separate the fish ?
How do I get rid of the disease in my main display after all of that?

You need to provide the video first. 20 seconds in white light. Posting to youtube and linking is the best route.

First thanks for all your help and fast respond.
Orange clown died couple of hours after this posts
A day after I managed to grab Polyb lab to use In display medic.
After couple of days black clown felling well , not seeing any spots or any type of sterss..
No more flickering and heavy breathing..
I'm thankfull that's I have managed to save 1 of them atleast
Polyp lab - I was negative at first but now, I can't deny that it didn't even upset my corals and it's works great

Unfortunately I do not believe that will eliminate the disease in your system. It may add some temporary bandaid relief but the problem is still there. QT and fallow tank is what will be needed.
