I need help identifying what is wrong with my fish

I went and bought a 30 gallon QT tank. I put 20 gallons of water out of my main tank and 10 gallons of new water. I was able to catch my Nano. I did the freshwater dip and then added him to the new tank with copper. Fingers crossed for a miracle. She looks terrible but acts pretty ok.

Did any of the black marks come off during the dip?
 
OK! So if the black dots were coming off during the dip then it's likely you're dealing with black ick. That's treatable with Prazipro! Yay!

Now for the bad news... I do believe he's fighting an infection as well. If it's a gram negative then you'll have to treat fast and hit it with Kanaplex, Furan2 and Metroplex. I think I'd go with the antibiotics first.... minimum of 10 days straight following the dosing directions on each package. You may need to keep it going longer if the infection hasn't cleared up by the 10th day.

I have and idea but I've never tried it, and @Humblefish can give his opinion on this but - you might be able to replace the Metroplex in that trio with General Cure. This contains Prazipro and metroplex in powder form. If that's possible then you can address both the infection and the black ick at the same time.

Mixing Prazipro with other meds can be dicey. I think what I would do in your situation is dose Prazipro to treat the worms, and then in 24 hrs run carbon + do a large WC to remove it. 24 hrs after that, remove the carbon and start dosing the antibiotics @melypr1985 suggested.

First thing you need to do is remove the copper you dosed. It isn't applicable to this situation and will just make the infection worse as copper is an immunosuppressant.

Thanks for the clarification on that! Figured it might be iffy to use general cure instead of straight metro in the mix
 
I appreciate everyone's help so much. My Naso didn't make it. She died yesterday morning. I will have myself armed with all the necessary stuff, just in case there is a next time.
What do you all recommend I have on hand?
