I think my hammer is dying....

I need some serious quick advice as to whether there is anything I can do for my hammer. I got it Wednesday of last week and it was doing wonderful until about 4-5 days ago, it started to shrink in and wasn't expanded fully. I've had it on the sand bed the entire time to acclimate to the lighting. It wasn't fully expanded this morning but it wasn't horrible either. I just got back home and it looks like the polyps are falling out or something.

Mg 1110 (I asked in another thread regarding the low Mg and was told that it is more than likely a false test and is higher than this.)
Kessil a360w-e light that is on the 8th day of a 10 day acclimation period.
Please tell me there is hope for this hammer, it is my favorite and it was so pretty.

Salinity is high, 26 is the upper limit.
Nitrates are high, bring them down..
Alkalinity is low, try to shoot for 9ish
Bring the Mag up as well..
Unfortunately it does not look well.
How are you testing?
What kind of flow is it in now?

Salifert test kits for everything. I have a jebao rw-8 on its lowest setting and I also have a split return that I use for flow. The weird thing is that although everything is low, it's always consistent there hasn't been any swinging back and forth. My corals have been doing good up until beginning of last week I think. I have another hammer which is also mad but not as bad as this one and a green toadstool which hasn't opened up yet since I got it two weekends ago. Aside from that I have an acan which has sprouted two more heads, orange plate,trumpet, Duncan, green nepthea, and two zoa colonies. All of those are doing wonderful. About to do a water change right now, that's all I know to do at this point. I'm just confused as to how it could decline so rapidly from this morning, and this being the hammer that was better of the two.

I would start by trying to correct the imbalances I mentioned earlier.
What salt mix are you using?

IO reef crystals. Just finished a water change and am getting another ready for tomorrow. The piece of tissue that was hanging off in the last pic fell off, the rest of the tissue appears ok for the time being and I want to believe that it's actually expanding a tiny bit, but that may very well be my imagination. Here's to hoping it at least makes it through the night....
