Ich or Brookynella?

OH no. Found my clownfish laying on the bottom of the sand this morning. Did a 30% water change but then I noticed these white spots or splotches. Really scared now, hermit crabs and snails are all fine though. Clown has also not been eating for the last 3-4 days, but I only noticed the white spots today. Pic:https://imgur.com/YeOebEI HELP!

How old is the tank?
What was the last livestock (fish, invert, coral, etc.) added to the tank and when?
What sort of quarantine procedures have been used with and around this tank?
