Ich or fighting marks?

The tang has been in the tank for a few weeks and today just all of a sudden these appear. Anyone know anything?

Are you talking about the three sort of dark spots? Are they just on the fish's left side? Not likely ich, could be a skin reaction to flukes, but I see another tang in the background, so it could just be from fighting. If you want to take a deep dive, look at this link and answer what questions you can about your tank and we''ll see if there could be other issues:

Okay so its a 75 gallon reef tank total volume. I run 2 filter socks in the sump with a refugium and a octo skimmer. I also have a 15w uv and run a mini carbon reactor. There is another tang in the tank as well as a Melanarus, 2 clowns, and a fire goby.

Nitrates and phosphate have always read 0

The affected mimic tang has been in the tank for 3 weeks after 2 in qt. The tomini beat him up the first day and I added a mirror to the side and he was preoccupied but since that they have been friendly.
 
I worked a double last night so I didn't do my usual inspections so it is possible it started yesterday. The wife didn't notice it tho
 
Also yes the dark spots are on just the one side however the bottom fin is dark and has a hole torn in it

I think it is from fighting, especially with the hole in the fin. You will likely not see it happen - a tang squabble can be over in seconds, and may only happen when you aren't in the room. That said, it doesn't mean the fish could have some other issue - two weeks is a very short quarantine time, and I would worry about flukes.

Update:
The dark spots on the tang are just gone. I dosed a 7ml of API stress coat and withing 24 hours it just disappeared
 
Update 5/2

The tang was fine up until this morning didn't eat and suddenly saw it with the cleaner shrimp and at a closer look the shrimp was eating him, not cleaning him. The tang is dead.
