I'm unsure if this is ick or maybe something else. Unfortunately I somehow missed the spots until he was already in my display tank, and I don't have a QT set up yet (I guess I'llbe going out tomorrow).

What are my treatment options? Am I going to have to treat my other two (currently healthy) fish or go fallow to fix my mistake?

Do you have a pic? If you put any fish or anything wet in your tank without correct/proper QT or running the non fish item to be added fallow itself then you’ll have ich in your system. Even fish that appear to be clean and healthy, if they haven’t been properly treated and kept in ich free water then you will most likely introduce ich to your system

Already posted as you were writing your comment. I also understand how ick is introduced to a system, I'm asking if I'm going to have to go fallow because I can't remove the fish until tomorrow, and maybe what methods people prefer for treatment in the QT tank.

It looks like it got ich. If you want to QT, then you need to take out all fish and QT them with meds. Leave the tank fallow for 76 days before adding the fish back in. If you don’t want to QT, there’s a method to manage ich by feeding high quality foods with selcon/garlic.

I have the same fish with the same spots. No issue on my end. May just be something g with that species. Mine looks identical though

So it wouldn't do much good to just remove and QT the goby at this point?

Unfortunately no, ich is already in your system with all over the rocks and fish. You would have to take ALL of them to QT or otherwise QT is pointless for just one fish.

Definitely looks like ich. Sorry wasn’t trying to condescend. But yeah once he’s added you’ll have to go fallow if you want to be get ich free again since there’s no way to know which faze each cyst is in. The moment it hits the water ich could fall off and into your tank. I prefer tank transfer method personally. It’s a bit more work but definitely more versatile Incase you run into other issues like a bacterial infection you can treat at the same time as opposed to copper which conflict with a lot of meds and hypo which typically leave marine meds ineffective. I’ve also had better luck being successful eradicating the ich using TTM over the other two

No but you could add something like kordons ich attack. It’s reef safe and it helps the fish cope with the effects of the ich and helps it recover the breakout on its own. You’d still have ich in your system but a lot of people are successful using some of the ich management methods

I recently lost two of my tangs to ich and my lfs recommended kordons ich attack just to help the other fish for the mean time. Also thinking about starting a qt tank.

It definitely helps. It is in no way a cure for ich though. It is strictly just to help the fish feel better
