I brought home this bristletooth tang and I didn't notice anything in the shop. He was fine all afternoon and night. Next morning he was gasping on his side and dead within an hour. He was in Q tank, but had been feeding it, double checked for Ammonia = 0 and Nitrites = 0 and Nitrates like 2-3 maybe, temp 78, salinity low 1.019 to match the system he came out of.
saw these white spots on the body near the belly. Store is replacing on next shipment no questions asked but I'm worried he may have left something behind in the Q tank?

It’s hard to make out anything in the pic (out of water is always hard), and most parasites leave after the death. Based on the timetables, velvet seems like a more likely suspect.

It is very difficult to assess a dead carcass especially after 2 hours unless sample under a microscope and it can be flukes, ich, velvet or toxicity if you have false test readings
 
What should I do to make the Q tank safe for next tang, If I do 100% water change is that enough or can velvet and/or ick cycle through the rocks?

You will likely be dosing with copper, so water change yes and bring therapuetic level back up and follow protocol

Yes, the rock is likely infected and would need to go fallow or not be used in your DT if it has been exposed to copper.

I would get some new biomedia and use a bacteria starter and begin again. Give the fish a day or two to recover from the transition from the LFS and bring the salinity up to 1.025. Make sure you have a good (non API) test kit for copper and use an airstone. I would also purchase Hydroplex and give the fish a 10 minute (saltwater) dip in your acclimating water before putting him in your QT tank. This will kill any parasites on his body. Then follow Jay Hemdal's quarantine procedures:

how long has qt tank been setup? How did you set the tank up?
It's been set up maybe 6-8 months. I took LR from my DT when the first went through it. I've had like 6 fish go through it now, all without treatment and without anything popping up that I could see. No fish has ever shown signs of disease in DT but of course it could still be under the radar. Not all fish made it in the DT, one was given away due to it being attacked, one was returned to store because it rapidly become the attacker, and one jumped to his demise in the DT. They all spent a month in there before the DT with just observation. The tank hasn't housed a fish in 6 weeks, just some snails and Chaeto but I dropped pellets in every 2-3 days to break down and feed bacteria (and Chaeto I am growing out). I thought I might vacuum the gravel in DT and use that and DT water to replace QT water and add a dose of Dr. Tim's Re-Fresh for good measure hoping that between the gravel and that the bacteria is boosted. But again, I tested twice for Ammonia and for Nitrite and it wasn't there, I'm pretty sure a disease killed that tang. In addition to the White spots it was very thin, and it was hiding under a rock in the LFS.

Over how long a period to bring the salinity up to 1.025? The rock hasn't been exposed to Copper but its LR from my DT sump. The LFS treats for 3 parasitic infections upon receiving a shipment and holds fish for like 4-5 days before being for sale, I will call tomorrow and find out exactly what they treat for and compare to the above protocol.
