That looks like tang fingerprint disease. Here is a link to a similar case:

Hi I have this Juvenile blue Caribbean tang I've never had a juvenile fish before and im not sure if this is damage or he's changing to his blue colouration? Should I be concerned

Sorry - I don't know the cause of this.

Atlantic tangs can also get a "brown spot" disease that is caused by a trematode in the skin, but those spots are smaller.
