Looks like hlle (head and lateral line erosion), often brought about by poor diet or poor water quality.
Check params and list what you feed it. The scars may never go away, but can be healed up a bit.

Very heavy case of hlle which is erosion and often caused by poor water quality and/or diet
While there is no formal treatment for this, maintaining utmost water quality help with what will be partial healing and coloration
Foids should include amino and sponge:
LRS herbivore diet
Hikari angel formula
Hikari veggie diet
Formula 2 frozen
Mysis shrimp

There's food that contains sponges that angels will benefit from. I can't remember the brand right off the top of my head.

Do you run carbon and how do you use it. If it's in a reactor that may be your problem. Fine carbon dust is said to cause hille.

You say the fish has been this way for several months? If so, has this gotten worse in that time? As in have the patches gotten larger?

If not, that is good thing, as it means the progression has stopped. And this may just be scaring that is always there.

If it has gotten worse, then if it is HLLE (I am on phone and cannot really tell), the fish will need to be fed as much as it will eat and fed very nutritional stuff (looks like have that part down)

Also, I would steer you away from carbon pads. They will release a fine dust throughout their use and that is what is usually associated with HLLE from carbon in tangs.

Did you rinse the pads off well or notice dust coming off of them in the water when you had initially put it in?

I have not ran carbon pads in many months and presently not running it. the last time was to aid in removing the copper I was dosing. as to the progression of the areas it has not changed in shape or size since I noticed it. so that is a good thing.

Sorry, that is severe HLLE. It just takes some previous carbon used/exposure and once it gets started, stopping carbon use doesn't seem to help much.

I've never been able to reverse it once it gets that bad. I think the best you can hope for is to stabilize it with good diet and good water quality. However, that does not mean that poor water quality or poor diet is the cause for this, that has never been proven.
