Any help on diagnosis? He came from Petco. Not that it matters, but we typically don’t purchase from anyone besides our lfs. Thanks for the help.

Looks like he has ich, possibly flukes. Some of the spots appear to be where they have previously fallen off and might turn into a bacterial infection

Is this a pic from the store or in one your tanks?

This is a case where a visible ID just isn't possible. Most of what I can see are mucus plugs. These can be caused by a variety of factors, some known, some unknown. One of the causes is marine ich though. If that is the case, your other fish are at risk.

Can you give us some more history:

How long have you had the fish?
How long ago did the spots show up?
What other fish and animals are in the tank?

It’s a 2 month old tank. The fish is the only inhabitant, besides a hermit and snail. It showed the signs about 2-3 weeks ago. His behavior has been fine.
 
Here is what I would consider doing depending on the size of the tank.

If you want to keep the rock place it in a fishless environment for 80-90 days with the CUC. Trash the sand, since there’s no positive ID place the goby in a hospital/quarantine tank with bacterial meds, general cure and transfer to a sterile tank every 36-48hrs
