I don't know, it looks awfully large to be a parasite. It may just be healing damage. That's a strange spot to see "black ich" (and have it be the only spot) nor does it look like black ich or a parasite to me anyway.

I wouldn't worry too much from what little I can tell but again I'm not an expert.
It would help to know how long you've had him, his other tank mates, and what new additions you've had in the last 30 days or so.

^^This would be my guess as well. It looks too large to be a parasite or worm, such as black ich. I would just monitor closely and update this thread if his condition worsens. One thing your wife can do is start soaking his food in vitamin supplements (ex: Selcon, Zoecon, Vita-chem) to expedite the healing process. It would be unusual for a clownfish to get an infection due to their incredibly thick mucous coat, but one never knows.
