I have no idea of the name of this LPS. This was my first coral I got about a year ago. Anyone have any info on it??

Looks like goniopora or flower pot coral.

12 tentacles would indicate that it is an Alveopora.

Agree it's an Avleopora, as the polyps become more extended you will see the tentacles have a much wider look than goniopora. More like a flower petal.
