A higher resolution/closer picture, a description of its behavior, and information about where/when you found it would help to better identify it. Can you get a closer picture, especially one where the “head” area is clearer? Haven’t seen anything like that before. You may try looking up pictures of marine leeches, but honestly can’t tell until more information is given.

Looks like a medusa worm.

Medusa worm. They are reef safe but release toxins if damaged by powerhead or pump.
