Oh my goodness. Bad case of Bacteria and brooklynella. ASAP, enter this fish into freshwater same temp as tank for a 5 minute dip which provide temporary relief.
Thereafter, immediate treatment needed with Formalin (37% formaldehyde). Product such as quick cure and Ruby Rally Pro will contain formalin with quick cure likely more effective and administered in the form of a formalin bath daily.

My tank is 4 months old, mixed reef, been battling algae issue, but otherwise parameters all look good, other fish appear healthy - just have another percola, and a cardinal fish. Running carbon, skimmer, floss, gfo

how would brooklynella present vs itch?
Most similar symptomatically to Oodinum/ich, this is also a parasite that primarily attacks the gills first. in the beginning, fish may scrape up against objects, then rapid respiration develops, and the fish often gasp for air at the surface as the gills become clogged with mucus. thereafter, the fish will become lethargic, refuse to eat, and its colors will fade. The most noticeable difference that sets Brooklynella apart from Oodinium is the heavy amount of slime that is produced by a fish that has contracted this parasite. As the disease progresses, a thick whitish mucus covers the body. This will usually start at the head and spread outward across the entire body.
