seriously though it looks like it is missing its "head" section maybe, so it could still be alive in your tank, i would be scared of ever going near that tank. Hopefully someone knows an id because it looks like a mutant fire worm that got half flattened.
 
Almost looks like a type of nudibranch, but I haven't the foggiest clue. Closest I can find for related images is a species of Polyceridae

Very close to an acro boring red worm, but its not. Im thinking a worm like fireworm missing part of body
