This little guy seems more lethargic than normal and I noticed a little bump on his head is this ich? Treatment?? Or gas bubble??

This looks like a result of injury or mucus cell. I would recommend in a separate tank treating with Seachem Neoplex.
Is fish still showing good appetite?

I don't have a separate tank. Appetite is not great just sits there when I feed Mayne picks a little but not like he used 2

You'll need one. Many life saving medications for fish contain copper that is extremely deadly to inverts (so all your snails, corals, crabs, etc...). That's why a separate tank is necessary if not for quarantine, at least for emergencies. For now maybe find a local reefer who could help you with this?

I recommend treatment. A quarantine tank can be as simple as a tank from a second hand store, a rubbermaid type tub or a starter kit from Walmart which most of the needed essentials.
 
So can I just buy a small tank with no flow? Obviously correct salinity and temp??
 
You need flow to provide oxygen exchange, it can be an air pump, don't forget to buy the stone and air line.

Petco has their tank sale going so you can grab a 10g cheap

Can you tell if this is fluid filled or gas filled?

There is a rare condition that causes fluid filled vesicles in fish - the case isn’t known, but is probably viral. There is no cure and fish can die from it.

If the lesion is gas filled, it could be from gas supersaturation, or another issue called gas bubble disease.
 
I can't find neoplex?? Any other recommendations? Api melafix?

Melafix is just a tonic, not an antibiotic. Can you get any TRUE antibiotics? I see you're in Canada, you may not have access to aquarium antibiotics.....
