I reintroduced fish slowly over a week and a half. My blenny disappeared three days ago. I thought nothing of it because I figured he was cruising the holes in the rock work. I finished adding my last fish my powder brown two days ago. I came home from work to find him like this. Both eyes gone. My assumption is crabs were starting to break him down, but I'm at my wits end. I feel like giving up. PH is 8. 3 temp is 78.5 amm is 0 nitrite is 0 nitrate is 5.

I feel you here, I am going through a tough period with my tank as well, but the great community here should have some advice and get you back on the right path. Don't give up, a beautiful tank makes it worth it at the end of the day.

Sounds like you were doing things right. My only suggestion is to try to use copper power with a Hanna high range copper checker. You will know for sure that the copper level is right.

Unfortunately, 10 weeks is just short of what some strains may live. The other possibility is if at any point during QT the copper level dipped long enough for one to become reinfected.

In addition to dwest, you want to ensure that your copper level never falls below therapeutic levels. If it does, you unfortunately have to restart your timetable. I have a hanna copper checker on the way for this very specific reason.
I made sure it was at 0.4 for the four weeks and always adjusted the water change water to 0.4. Whatever is happening isn't ich it wouldn't kill a what appeared to be healthy fish in two days. I'm going to check ammonia with another kit to double check that there isn't a cycle I still have two skunk clownfish and a coral beauty left.

Is it possible you had velvet present prior to QT?
We're you using a visual test method or the Hanna checker?
It's fully possible velvet remained on your fish and was able to cycle in the QT unseen due to the copper presence. If the level dropped slightly below .35-.4ppm it could have happened.
I just don't see how it would fly under the radar. Cupramine treatment was the first thing I did followed by another 6 weeks of visual inspection with no copper in the water. If it was going to present itself I figured it would have long ago

You are correct with 6 weeks of observation it's likely that you would have seen it show up while still in QT.
This leads me to believe that it has something to do with your DT.
No worries we understand the stress and most of us have been in your situation at one time or another.

Powders can stress to the point of death, just as with any fish. Sounds like you did it right. Sometimes they just die. Sorry for your loss. Hang in there. We've all been there and know how it feels.
