Internal parasite or just well fed?

I’ve had this molly miller blenny for over three years and over the past five or so months, he has become quite bloated. I always thought he was just getting over fed, but his belly is huge now and I am starting to wonder if it could be some sort of internal parasite. Other than his bloated belly, he is perfectly healthy. What do you guys think?
(Also on a side note, this is the only fish that I never had in QT, but he spent his early life in a tank with just him & one clown for the first two years)

My blenny was the same way after I got him. I started feeding dr Tim’s internal parasite caviar and it went down after a few days. I still feed the caviar from time to time for my wrasses. My lineatus started having sunken belly despite eating like a big and I’ve seen him start to put on some weight.
 
If you suspect internal parasites, another solution that has worked very well for many - including myself - is equal amounts of Seachem's "Focus" binder and API's "General Cure", which is a powdered combination of Praziquantel (for worms) and Metronidazole (flagellates and other protozoans). Sprinkle over a tablespoon or so of food, and feed everyone.
