I'm not sure if I see what you are referring to in the picture. Can you describe it? Or maybe try to post a clearer one?
 
Does your tang eat a lot of nori?

Sometimes their gut can look funny when they do
 
I really don't feed much nori. The area below his pectoral fin looks lumpy, and is wider than the rest of him when looking head on.

That is odd. Have you seen him poop? Is it white and stringy or brown?
 
I can't say I've noticed any stringy poop...

Definitely watch for that as it's a sure sign of intestinal parasites. Until then, you could go ahead and treat for them by feeding metroplex + focus in the food OR we could start with feeding a pealed boiled pea in case he's just constipated.

Does he eat sand? I know some of my fish poop out sand
