I might need help – there are days when this guy looks huge and wonderful, but the past couple he is looking a little lack luster. There seems to be less tentacle stuff in the middle… I do not know what kind of coral this is or what it needs and it was fine for a while – I am running a protein skimmer, all of my water tests seem fine. Maybe circulation? Maybe the lighting? Please help so I can save him!
 
That is a Fungia aka plate coral. It definitely looks upset. Is that Cyano on the right lower side? Looks like you have him in a macro algae bundle...

And before it gets asked, you will need to post up ALL of your parameters.
 
True story all params please. I don't see cyanocobalamin but I may be blind just looks like black substrate to me . Long tentacle fungi "plate corals" are lps. They are a little more intolerant than there short tentacle counter parts when did you get him . What kind of flow in the tank. Tank dimensions and lighting please.

Oh my gosh- I got home and he is so much worse!! WHAT is happening!?! Can I save him?
I can't test the water with a fancy test kit- the ONE saltwater store it over an hour away and it it ten at night.... HELP! I am sick with worry...
 
Heliofungia are a bit more sensitive to water perimeters than regular fungia. IMO once the tissue is gone up to to mouth. It may be too late. But i would move it to a low light area. Until you can get water perimeters checked. It looks like alot of macro algae near it. Generally enough light to sustain macro algae may be too much for a plate coral.
 
It is on the bottom of the tank, so I keep the lights off?
I don't know what macro algae is- I can take my water in for a test on Monday, hope it is not too late-
Should I do a water change?
 
The macro algae is the plants they are referring to or is that a plastic plant?
 
If the flow is causing the green blade of algae above the fungia to hit it that could be a reason for its death. It could be a number of things outside poor water quality. If your lucky and it was happy before it's death maybe you will get babies on top of the dead skeleton.
 
The plant hits it constantly- I thought it was neat and snuggled in- I am going to yank it right now-
 
No need to yank it just move the coral, lower flow and light might help coax it back. I've lost a beautiful acan colony From the same thing. If your water quality is good it may not be to late. Theoretically as long as it has part of its mouth it can fully heal.
