I try to take the best pic as i can. Thanks for any input.
 
I am no expert, but looks like ICH to me. I have seen velvet and it looks more of a smaller particle dusting.

There is also few larger dusty stuffs that looks like it can fell off of his body ( under his belly)
 
The dusting I am referring to is much finer. I would put my money on ICH. Cu will be your friend for the next couple of weeks.

That is velvet with that kind of coverage. Ich spots are generally larger in size and fewer spots.

Recommend a 5 min Freshwater dip for temporary relief followed by a bath in acriflavine (Ruby Reef Rally) to prevent secondary infection.

Then you will want to treat the fish in QT with copper or chloroquine phosphate (CP) for 30 days.

Also you will see in the above link, time is of the essence, you really want this fish to be in fully therapeutic copper or CP within 48 hours to increase the survival chances. A FW dip may buy you a little more time if you don't already have meds on hand.

I stand corrected, I would go with the pros ^.

The velvet I saw was definitely more fine, but my experience with fish pathology is very limited.

Good luck with treatment and keep us updated on how the fish does.

Keep in mind, the Acanthurus genus of tangs have a very thin mucus coat. This makes them especially succeptable to parasites. You will often hear people refer to them as a tangometer in QT. Velvet can vary in appearance from fish to fish. For example Wrasse are the opposite, very thick mucus coat. They often harbor velvet with no visual symptoms at all, sometimes only behavioral symptoms. Just my .02
 
So, i treated it in QT with CP, today all of spots gone and he is eating like pig again, at this point i think he will survive that i’m so happy.
