Is it time to give up? A vent

I actually don't see any ich on the long nose, but that does not mean it's not there.

Could it be flukes hiding in the gills? Did you do a freshwater dip when you got the fish before adding it to the MD? Unfortunately, a dip now would probably push the fish over the edge. If the fish dies (likely...sorry), than put it immediately in some freshwater for 10 minutes and see if any flukes come off. Google freshwater dips on saltwater fish and watch some videos so you know what to look for. You'll have to be quick about it though because I don't think flukes stay on a fish that has died long.

I'm am sorry you're going through this. IME, many LFS are more interested in a sale than in the health of the fish they sell. :(

Exactly. My LFS said he would fit fine in my tank, but once I did more reading (after taking him home unfortunately) I called them back. They told me to keep him and see how he does. He ate fine but now they’re refusing responsibility for causing this Ich with this new fish and for recommending a fish in a tank far too small. Sigh.

Ya..LFS isn't a good resource most of the time. Smart phones are though.

I picked up a copperband and did a peroxide dip followed by NFG dip then into the conditioning tank and feeding live worms. So far so good.

Always pays to research fish and be prepared before buying them
... especially with more sensitive species.

An impulse buy can be a total disaster... especially for people newer to the hobby.

Don’t give up yet. Ich is manageable. As a matter of fact I am currently treating my purple tang from ich. I don’t QT any fish but since my purple got ich, I have decided to set up a hospital tank. It is not the biggest since I don’t have space in our tiny apartment. But because I like the tang so much, I have to do something. Not only that, I just experienced having power outaged while out of town and killed four of my wrasses. Saying this to let you know you are not alone.

So sorry. I know how you feel. Velvet wiped my tank out. Did my fallow (even added a couple of extra months), cleaned tank, new rock, and bought a UV. Set tank back up, moved corals back in, bought a few fish (same as what I lost), and all was great. I lost a few of my new fish to jumping. Fast forward a couple of months and added four more fish, three I quarantined and the fourth, a Hippo Tang, that was quarantined for me. Within two days, the tang started showing signs of Ich. I immediately took him out and brought him back to LFS (I hadn't had a chance to clean out water from last quarantine). I tried the Polyp Medic, but my BTA did not do well with it, so I stopped using it (not before losing some corals). It's been three months, and Ich hasn't shown up on my other fish, a Whitetail Bristletooth Tang, one male two female Lyretail Anthias, 3 Picasso Clowns, 1 Flame Hawkfish and 3 Barnacle Blennies. Unfortunately, I did lose one of the blennies, but never did figure out why. As of this moment, I have a beautiful reef to appreciate every day.

Moral of the story? Don't give up!!! There are highs and lows in this hobby, some we can control and some we cant. I was ready to give up, and one night, while sitting in front of my tank, I realized it was worth the risk. We have a small piece of the ocean in our homes. All we can do is do our best, learn from our mistakes (ALWAYS have your quarantine ready to go BEFORE buying new fish) and appreciate the beauty we are blessed to have in our homes. Luckily, we have R2R to help us all through it all!:)

From pics, I see possible bacteria issue on body as well as spot on body.
My success has been use of Melafix with such issues, but I ask age of tank as these fish dont fare well in a newer system and not one of the easier butterflies in a new tank to keep happy. Once the fish is eating and established, its a whole different fish.
Once eating, It should be fed a varied diet of meaty foods such as LRS foods, mysis shrimp, crustacean meat, and even clam on a shell
