Not in the hobby for too long, but here is whst I'd do. Move it to a safe space and see what she does for 24 hours. If no progress or cloudy water or whomit, just take her out...

Definitely gone, please remove before it severely fouls your tank.

Anemones need very stable tanks, something not to add for months after setting up a tank. :)
 
Water salinity was a bit high and we added RO water like we’ve done before but then it did this. We’ve had it for over a year. It sucks. Fish and cleaning crew all doing good and all is good with the water test.

Only thing off was the salinity. We did a water change and added more RO to stabilize and it did that.
 
When salinity is off, everything is off and they are sensitive to water fluxes, like SPS.
Since he was not “new” (a year) I would agree that the change might result in what you see.
When changing salinity (or maybe everything for that matter) should be done real slow over several days.
Consider utilizing an ATO which will keep salinity in check at all times, salinity being the cornerstone of water chemistry.
 
I seriously doubt the salinity change alone would cause that, unless it was way off. Is it possible there were chlorimines in the new salt water and RO water?
