Is my clam dying?

I’ve been out of town for a couple days, and my dad was taking care of the tank. I just got back and noticed that my maxima wasn’t looking so great. It looks like it has a hole on the inside. Idk if that’s normal or not because this is my first clam. Can someone let me know if I’m wrong or not. Thanks
