I've seen my branching hammer look like that during a split.
 
Typically hammers, well euphyllia in general will form a bubble over the oral disk when they undergo a split. This process can take anywhere from 1-4+ weeks. One day you look at the coral and there is a bubble over it's mouth. Then seemingly weeks later it's gone and now there are two distinct heads visible when the coral retracts.
 
See I got it with two heads and I had it on the rocks was never happy added it to sand bed about a month ago and now it’s three possibly 4
