Is my torch dead or dying?

I think my Torch is dead. It was fine for the first 3 weeks I had it then I had a real bad phosphorus problem (and algae to go along with it) and it hasnt been the same since. Also I just noticed what looks to be a hole but im not sure. It has always had 2 heads so its not where one is gone. In the tank is also a clownfish, damsel, 2 snails, and a hermit crab. Sorry for the bad pics only camera I had was iPhone

I wouldn't say it is gone. The flesh is still connected to the stem. What are your water perimeters? Where are you keeping it as in flow and lighting.
 
I test the water every other day (well I take a sample to petsupermarket as it is next door to my work) and its always perfect, the salinity ranges from 1.022-1.024 usually. The flow could be better, I have a 10gal tank with a 20gal filter on it so its not amazing flow, but the filter is in the center of the tank and the torch is off to the left in the front. I would say lighting is okay its sitting in the substrate but again only a 10gal so the light is maybe 24" from the torch.
 
The salinity may be a bit low should be 1.024-1.026. If your other perimeters are fine, however for a place to say 1.020-1.024 is ok is questionable, there are other variables. Lighting and flow. What type of lighting do you have.

On another note not sure what damsel fish you have but that with clown in a 10 gallon tank may seen ok now but will have issues later.
 
Okay with my next water change here in a few days ill raise the salinity. And im using the ZooMed Aquasun T5 light fixture with a Coral Sun Actinic 420 bulb and a Ocean Sun 10,000k bulb. The damsel is just one of the small blue with yellow tails that wont get any bigger than 2", one of the cheap 4.99 ones. And maybe ill check into getting a small powerhead since it is only a 10gal or just move it to a higher spot with more flow.
 
It is not dead. Just give it time. Keep doing water changes and get your parameters in line. I agree with the above comment about salinity. You need to raise it some. I would also have someone test your water to see where the actual values are at.
