Is my wrasse sick or are these organs?

Hello

New to the hobby and adding a yellow chorus wrasse to my tank. When I purchased him from the LFS, I noticed marks on his head and towards the back of his head (see attached pic). They don't really look surface but more like below the surface.

I asked if they were normal and they said they were just his organs showing through. Can anyone judge from the pic if this appears to be true or if my fish is sick?
