Is something attacking my Favia Brain Coral?

I don't have a picture of this problem just yet. It appears that my Favia is running into some issues. It's skeleton is showing where one of the polyps used to be. I haven't seen anyone eating it, and all my parameters are good. Nitrates are sitting at 5, but everything else is good. Has anyone ever run into this issue? I have Coral Revive, would anyone recommend dipping it for a bit?
 
Happened to my Favia and bec of High Phosphate levels.

So many people jump on po4 so fast as a problem. Favia is an easy coral I have a few growing just fine and my po4 as of tonight...

If you look at the coral at the Nine o'clock position you can see the white skeleton showing. That wasn't there 2 days ago.
