Is this a bacterial infection?

I’ve had this sinularia/nepthea for almost 2 years and in the past few months I’ve noticed it hasn’t been opening as much and the base tissue has started to recede, how would I treat this? antibiotic qt? Coral dips? First pic is the coral fully extended, some of the tips were turning yellow and then black in the past and eventually they would fall off

I don't know if that's an infection or not.

I would be tempted to make a couple small frags from the healthy tips.

agree cut out and away from necrosis ,iodine dip frags ,SW rinse dip before adding frags back.

Along with parameters, especially changes in parameters: Any changes to lighting and flow?

last time i tested they were:
NO3: 25,
PO4: 0.1,
Mag: 1420,
Cal: 440,
PH: 7.9,
Alk: 7,
salinity has been stable at 1.025,
i know the nitrate is high, i have a refugium and skimmer and it still wont go down. like i said, this has been ongoing for a few months, i raised the flow in the area to see if it would improve but it didn't seem to help either.
