He was doing just fine 3 hours ago, now this! He is my first one, I have only had him a few days and don't know if this is a normal behavior or not!

It could be splitting. When they are actively dying they often detach and you see a large/gaping oral opening.

It hasn't detached, but it's mouth seem swollen/puffy all around it.

I would not be overly concerned right now. If the anemone has been inflating as normal than I would wait and watch.

What size tank?
What lights do you have?
How long has the tank been set up?
What are your current water parameters? (( with numbers please )).

Seems that was a false alarm. He is happy and super big today. I think I just didn't know what his program is. The tank is 75 gallons with a 100 gallon sump/refugium. I checked the parameters. They were all good, did a water change just to be sure. I did re adjust my skimmer last week, and it took a little of my salt out. I put it back to where it was and slowly an bringing the salt back up. I think we are in the clear, just gonna watch it for a few days. Thanks for the help
