Can you give us more history? Is the fish eating, water quality, etc.?
The fish looks thin and it has some head deformity (sometimes seen in captive clowns).

I wouldn't purchase from that LFS without doing alot of research. Other experienced reefs can chime in on the issue.

but was losing its color and was lethargic.
