is this hlle? help!

This is a friend's tang. He is eating normal and active. He does however have this spot and a big white spot on his side under his gills. He presents himself to the other fish like hes requesting a cleaning. All levels are 0. Please help!

Does he use carbon or have high nitrates? Those may be issues. Fish should be ok if it continues to eat

Kinda loos like HLLE. Get some selcon and nori sea weed. It may take months to heal
