I can't see the actual ich spots in the photo (tough to get clear ones, I know). The blanched spots in the skin can be a tissue reaction to ich, flukes or bacteria. More background information is needed; how long have you had the fish, how did the spots develop, how long has it had the spots, treatments you have tried so far, etc. This post gives information about how to supply all that info:
