Hard to see from the pictures. Is it just the one spot up by the front of his head?

Yes just 1 spot
Fortunately it doesn't look or act like ich. Unfortunately it's possible that its either lymphocystis or a bacterial/fungal infection. Has be been bullied by any other tank mates. An does it just look like discoloration or is it actually a raised spot? And is anyone else showing any signs of illness? #reefsquad

I agree with @crabs_mcjones with out a good close up I would lean more toward an injury:) I would still treat the wound.

Mysis once a day
I'm not a wrasse guy, but I think most people recommend feeding wrasses at least 2 or 3 times a day. What kind of wrasse is it?
@eatbreakfast what do you think? Does this guy look underfed?

If daily feeding is enough I would worry about an internal parasite.

i have a tank full of wrasses, i feed 2-3 times a day, i use a combo of LRS herbivore and mysis and i also do dried pellets...

as for the wrasse possible scale damage, keep an eye on it, its white so its still clean, if it stasrts to turn brown or fuzzy then a bacterial infection may be setting in.. honestly just leave it alone and watch it, it should hesal on its own..

I agree with the above, but that wrasse is very thin I’d feed that guy as much as he will eat, honestly.
