Visually identifying random white spots on a fish can be difficult. I don't think these are bubbles or random spots of sand. However, it could just be Lymphocystis, a non-fatal viral infection.
Let's assume the worst, and that it might be marine ich....
What other fish are in the tank with the wrasse?
Any spots on them?
Watch the spots on the wrasse - if the same spots stay in the same location for more than 3 days, it likely isn't ich. If the spots change in number, going away, but then returning in new locations and in different numbers, it is more likely to be ich.
Do you have a treatment tank available?

The wrasse lives with 2 ocellaris clowns, which dont seem to have any spots on them…
I’ll keep an eye on the wrasse looking for the things you mentioned.
As for an emergency tank, I don’t have one, hence why i began to panic

not at all, theres only two clowns, and one wrasse, and no aggression to date.Also the cloudiness you mentioned is only visible when looking at him from one side.I don't see ich spots on the clown - but there is minor fin damage and a general cloudiness to its skin. Could it be getting picked on by other fish?

The spots moving around and changing in number does tend to point to ich on the wrasse. If it has ich, then the clownfish and any other fish would need to be treated as well.not at all, theres only two clowns, and one wrasse, and no aggression to date.Also the cloudiness you mentioned is only visible when looking at him from one side.
And an update on the wrasse, i observed that the white specs have different locations now, so does this confirm it is ich?
Also, if it is, why hasn’t it spread to the clowns yet?

I have a similar question for my SS puffer… water parameters are perfect and temp is 78. She likes to lay under a rock structure in the sand and plays in the sand so I thought it may be sand stuck to her mucus coat… but as a new puffer fish owner I’m struggling. She is acting and eating normally other then tucking one of her fins into her gill (which I read can also be normal for puffers?)… help…
Yes I was asking if it was ich- unfortunately I don’t see a link but I will add 0ppm nitrate/nitrites/ammonia and ph around 8.3. Tank is about 4 months old and cycled. 2.5 gal water changes done automatically each day. Tank mates are a watchman goby and a clown fish (neither have any spots/ sand stuck to them). She’s a baby- only about 3” long and in a 75gal tank with a octo skimmer and algae scrubber. Plans for a 250+ gal tank are in the works already as I know she will get large.Hi, welcome to Reef2Reef!
You posted some pictures of a puffer, but it wasn’t clear, are you asking if the puffer has ich? One of the pictures shows some spots on the puffers side fin so it could be ich.
We need to know more about your fish though. The link below my name here will take you to a file that describes the additional information we need to try and diagnose and come up with a treatment for your fish.

She doesn’t appear to be breathing heavy and does not go to the top of the tank (except for feeding time and food begging lol) and doesn’t swim in the wave maker or return pump stream…Hi, welcome to Reef2Reef!

I can see spots, but can’t tell if they are ich. The best way to tell is to see exactly where some spots are located. Then after 36 hours or so - see if the same spots are in the same locations, and no new spots have showed up - it may not be ich. If the spots change in numbers and seem to move around, it is ich.Oh salt is at 1.025- 1.026 and I use reef crystals now but am transitioning to neomarine with each 2.5 gal water change- she will only eat frozen scallops and shrimp but I soak them with vitamin m and garlic from Brightwell

I’m traveling today and may not be back online until tomorrow - but if it is ich, you’ll need to treat with copper or hyposalinity.
