I’ve noticed some patches on my royal Gramma. Very small on the top of her head. She’s acting completely normal eating and swimming around the tank. I took the Best pictures I could let me know what you think.

Looks like a couple of nicks/injury but can be lymphocystis. Neither are parasitical and requires merely Good water quality. Assure the fish is getting proper diet also- This is not a flake and pellet only eater requiring small meaty foods which have fats/omegas/vitamins.
 
I agree looks like some lesions. Prob from squabbles or rubbing on rocks.
Dang buggers. That is a pseudo or dottyback fish? They tend to be a little more bossy so to speak. Lol. I had a strawberry one or all pink one that was a terror in a 10g. Lol.

Only thing to add is to watch for scratching/flashing. This can be a sign is skin flukes and scratching can scrape the scales like that.

looks to be a gramma, and agree looks like nicks - what else is it interacting with in the tank?

It is a gramma. Oddly enough I had/have(?) a Firefish we bought a week ago and suddenly this morning has vanished. I wonder if the Gramma and the fire fish fought it out? There are no remnants of the fire fish whatsoever. I have two baby clowns, and a crimson shrimp. And then just the regular cleanup crew. I’m praying to God that the firefish is just hiding. She went through her first water change yesterday, but seemed to be fine afterwards, ate and everything. Woke up this morning the fire fish was gone, the fire shrimp and molted, he does this every 10 days, and the gramma has that crap on her head LOL what a day right? I hope the fire fish just appears and that whatever is on the grammas head heels!

Fire fish are really good jumpers, look around the floor below the tank….but if you have cats, it may be gone.

It’s a sealed tank, and I do have cats, one of them stares at the tank relentlessly all day licking his lips! Believe me I thought of that too, I asked the cat but he seems to say no lol
