It is hard to see, but from what I can see i would say yes it is ich. To treat either remove fish and treat with copper in another tank or feed well and hope the fish fights it off. Do you have any sea weed in it's diet?

Looks more like velvet to me. Get him in treatment ASAP!!!!

yeah it dosent look good, do a water change asap. from the looks of your rock it looks like the no3 po4 is up there. check your salnity, keep the temp lower around 78 then if the fish is eating try feeding food soaked in garlic that always worked for me , if u see ich getting worse then dont waste time take out the sick fish and treat them . its important that u do those water changed and make sure your RODI is on point

I would qt the fish. Especially if it is velvet as that will eventually get on all your fish

theres no need to qt hes the only one in there
temp is 79 lower?
salinity 1.020
rock color is from the light that rocks bone white all old rock couple years reef rock no cycles or anything 0 nitrite 0 ammonia nitrate 5 or below

Copper that is why you need to qt as it will stay in live Rock, sand even the silicone seams
