If he stops eating or if is hanging out by the powerheads it's probably velvet. My guess from the pic is velvet cuz there are so many spots.

There is a ton of info on this forum if you search for it. I'm not an expert but i have had ich and velvet before. Velvet progesses very fast. I quarantine everything now. Something you may want to research. It's less work in the long run

Thanks for all the replies. For the most part he’s acted fine. Ate this AM and has been swimming normally around the tank. Scratching against rocks a bit, which is why I really tried to get a good look.

I got him switched over to QT as soon as I saw it. Planned to do the TTM way to treat, believing it was ich.
Just from looking at the pic my money is on ich.

It does look like ich to me as well. I prefer using copper power because this helps eradicate both ich and velvet.
