Islands or No Islands in Aquascape?

Curious everyone's opinions on aquascapes in general. Do you like them better with islands/single rocks in the sand or without and just the main structures? I'm currently seeding my new setup with some of my older rocks as well as media in filter, but can remove and am slightly torn.

I prefer the first one, I like more natural reef scapes but it’s all very personal taste.
